About Us

Our Story

Rooted in Experience, Guided by Community

ChosenWell was inspired by the concept of chosen family, a powerful tradition within LGBTQ+ and BIPOC communities that reflects intentional connection, care, and belonging. The name honors those who have built support systems outside of traditional structures and deserve affirming, culturally competent care as they age. “Well” speaks to both wellness and being well-chosen — a symbol of dignity, quality, and compassion. Together, the name reflects a commitment to serving clients with intentionality, respect, and a deep well of support.

We understand that care is not one-size-fits-all. Identity, culture, history, and chosen family play a vital role in how support is experienced. That’s why everything we do is grounded in cultural humility, representation, and healing. Whether we’re offering care coordination, peer mentorship, or inclusive training for providers, our goal remains the same: to honor the whole person — not just their needs, but their story.

Our Mission

To reimagine aging through culturally affirming care, advocacy, and community connection.
